About this role

Join PACE Organization of Rhode Island as a Home Care RN, caring for older adults where they're most comfortable: at home and in the community. You'll conduct nursing assessments, develop and implement individualized care plans, manage safe transitions of care, triage participant calls, and mentor CNAs in the field, all as a vital member of an interdisciplinary team. Schedule is primarily Monday to Friday, 8:00 to 4:30, with rotating Saturday coverage roughly once every five to six weeks plus holidays.
